Questionnaires usually comprise of two broad types of question - open-ended and closed:

Open-Ended Questions

Open-Ended Questions ask a general question and allow the respondent to provide a reply in their own words

Example:

What are the main reasons you drink alcoholic drinks?  (Please use the space below)

 

Closed Questions

Closed Questions ask a specific question and ask the respondent to select from one or more 'response categories'.

Example:

Which of the following is the main reason you drink alcoholic drinks?  (Please tick one)

I enjoy drinking alcoholic drinks  
My friends drink alcoholic drinks  
Drinking alcoholic drinks makes me feel more sociable  
Drinking alcoholic drinks helps me deal with stress
