Multiple Choice questions give a number of choices (or response categories) and respondents choose one from the list.

Very Poor  
Poor  
Neither Poor nor Good  
Good  
Very Good  
After testing the question, you may also decide to include other options such as ‘I don’t know’. The question then, in effect, becomes a Multiple Choice question type.
