A questionnaire is the set of questions you will use to
collect data for answering the objectives set for the
project.
The questionnaire is a key
element of the Health and Lifestyle Survey. It must;
- provide data
that meet the objectives of the survey
- translate
sometimes complex concepts into 'indicators' or measures
that the respondent can understand
- provide
reliable and
valid data.
Although
seemingly obvious (and often neglected) the best way to begin
designing your questionnaire is to follow these two steps:
- Draft, discuss, amend
and (finally) agree the objectives of the survey.
- Discuss and decide how
you will use the information from the survey.
Both of these steps
are crucial and will require input from the various stakeholders
of the survey. Only when these stages are completed
should you begin to draft the questionnaire.
